The best way to do that is to go to the top of the cliffs surrounding the shrine and then jumping down to where the scroll was. Once you've done this, press Down on the D-Pad to heal, triggering a cutscene that awards you with the Yharnam Helm, and the Yharnam Vestments for the Kensei Armor.
